The three discs are labeled 4,5 & Comical Deforme Theater. Disc's 4 & 5 contain the last 4 Episodes and each have their own insert. Disc CDT contains 3 Episodes, 2 Music Videos, trailers and comes with an insert as well as an 8-page booklet featuring video cassette cover art and a 1994 calendar. While the sleeves of discs 4 & 5 have the Box Set Reference #'s the discs themselves have the Reference #'s of the individually released discs. Disc CDT has its own unrelated Reference #.
