The jacket states that this is "an interactive videodisc for analyzing the motion of athletes". It was created and designed as part of the Physics Education Program at Kansas State University. It is an instructional program intended for use in science classrooms to "enable students to learn more about physics, and to see how improvements in athletic performance are tied to the laws of nature". Nothing resembling a reference number appears anywhere on the jacket or disc labels. The "dead side" is a white non-reflective surface.
