The four episodes of the first 3x3 Eyes series, concatenated and with original trailers included as "supplementary material" at the end. Chapter 15, though listed on the jacket, does not in fact exist. Additionally, although the Japanese audio is described as "mono" on the jacket, this is probably incorrect.
